    Good book, poor TOC & index

    This is a very good book and I'd recommend it. The information is very useful and practical as the cover suggests. What isn't practical is the really poor table of contents and complete lack of index. Even a 15 year old version of Microsoft Word can generate these. After finishing the book there are specific items I want to refer back to and unless you remember exactly where it is, you have to sift through the chapters. There is no excuse for this.
