This volume contains the proceedings of the Fourth Safety-critical Systems Symposium, held in Leeds, UK, 6-8 February 1996. Until recently the importance of human involvement in the development of technology - other than at the direct operational level - was hardly admitted. Now, however, the unreliability of humans is recognised, as is the potential for latent faults to be introduced into systems by all those involved with them, including designers and strategic decision makers.
